The Madlanga Commission’s second interim report has recommended immediate disciplinary action and criminal investigations against senior police officers Shadrack Sibiya and Fannie Nkosi, as well as alleged political fixer Brown Mogotsi and Witness G.
President Cyril Ramaphosa received the report on the 29th of May, following the submission of the first interim report on 17 December 2025.
The final report is expected on the 16th of November 2026.
Presidency spokesperson Vincent Magwenya says the latest referrals from the Commission will form part of the work of the SAPS Special Investigations Task Team established after the President received the first interim report.
“The report revisits the recommendations and referrals made in the First Interim Report in respect of the South African Police Service (SAPS) based on prima facie evidence and, with one exception, confirms these recommendations now that the relevant implicated parties have appeared before the Commission to give evidence.
The report also sets out new recommendations and referrals made by the Commission based on evidence heard since the start of 2026.”
Magwenya says the commission also recommended that the Office of the Inspector General for Intelligence conduct an immediate investigation into the off-the-books operation of Crime Intelligence allegedly involving Mogotsi and Witness G.
In the first interim report, the commission found prima facie evidence against several senior officials in Ekurhuleni.
Among those implicated are suspended Ekurhuleni Metro Police Department (EMPD) deputy chief Julius Mkhwanazi and former Ekurhuleni city manager Dr Imogen Mashazi.
The presidency previously said that only the final report would be made public.
